ACCESS — Advancing Chronic Care with Effective, Scalable Solutions — is a 10-year program built around a simple but consequential rule: pay for managing chronic conditions, and pay more only when measurable health goals are reached. In other words, reimbursement is no longer anchored primarily to activity volume. It is tied to outcomes. That matters because outcomes-based payment is one of the few policy mechanisms that can reliably pull AI from the margins of healthcare operations into the core workflow.
That is the part much of the tech world seems to have missed. The program is not a generic encouragement to “use AI in healthcare.” It is a federal-scale operating environment in which software has to help clinicians, care teams, and patients actually move metrics — glucose control, blood pressure, adherence, follow-up completion, hospitalization avoidance, and other measurable indicators CMS can audit. If the systems work, they do not just fit into healthcare. They become part of how healthcare gets paid.
